All sessions are by appointment only.
If this is your first appointment, please arrive about 10
minutes early, so that you can complete the necessary paperwork.
Please be sure to bring your insurance card. In many cases,
we can obtain any needed authorization from your insurance
company on your behalf. If you needed to get an authorization
from your insurance, you must bring the authorization number
they give you when you come for your appointment. Without
it, we cannot bill the insurance and will then expect payment
from you. Copays are requested at the time of each session.
We do not accept credit or debit cards.
Reminder - We make every effort to see you at the time of
your appointment, so please be prompt. The scheduled time
is reserved for just you. Please help us with our scheduling.
To avoid being charged for a missed or late cancelled appointment
we require 24 hours prior notice for a change or cancellation.
